Full Time | $18 to $21 per hour
Phoenix Home Care & Hospice is looking for an organized and dependable Care Coordinator to support the daily operations of our home care services. In this role, you will help ensure caregivers are scheduled appropriately and clients receive the care they depend on.
Care Coordinators play an important role in keeping care running smoothly by supporting field staff, communicating with clients and families, and maintaining accurate scheduling and documentation.

What You Will Do
Coordinate daily caregiver schedules based on client care plans and staff availability
Support and communicate with caregivers working in the field
Respond to client and employee questions or concerns and assist with resolution
Maintain accurate documentation, records, and reports
Assist with quality initiatives, staff communication, and training support
Provide professional and courteous communication with clients, families, and team members
What We Are Looking For
Must be at least 18 years of age
High school diploma or equivalent required
Previous office or administrative experience required
Strong communication and organizational skills
Ability to multitask and stay organized in a fast paced environment
Professional, dependable, and team oriented attitude
