Lead with purpose and help transform how care is delivered. Community of Hope is seeking a Lead Nurse Navigator to guide a team delivering high-quality, patient-centered care coordination for individuals with complex health and social needs. You’ll play a key role in improving outcomes, reducing hospitalizations, and helping patients navigate the healthcare system with confidence. This is a full-time position located at our Marie Reed Health Center in NW DC.
