• Supervises/participates in CCP Meetings - randomly attending at least twice a week to ensure accuracy of information presented. Provide guidance and efficiency to ensure care plan requirements are met.• Review charts and complete MDS of all Medicare residents. Conduct interviews (according to MDS guidelines) to ensure information captured is accurate (an average of 4-6 MDS per day is expected to be completed).• Complete MDS/CAAS as assigned by the MDS Director.• Review the MDS, CCP and CAAS from all disciplines and resolve any discrepancies. Notify appropriate discipline if appropriate care plans/CAAS are not present.• Assist with necessary completion of Nursing portion of the MDS in the absence of the Unit Nurse Manager. Complete the CAAS in the absence of the Unit Nurse Manager.• Maintains the MDS RUG score logs.• Review the PDA's ADL care plans and communicates with the interdisciplinary team when discrepancies are found.• Participate in Medicare meetings in the absence of the MDS Director.• Other tasks as assigned. Qualifications
Education Requirements
»Bachelor's Degree and current license as a registered Professional Nurse in New York State.
practice Requirements
»Minimum of two years of MDS or managery Nursing practice in long term care.
