Overview:
The Employee Nurse Care Navigator performs a clinical, specialized, and mobile role designed to improve the health and well-being of the Covenant HealthCare employees while balancing a focus on reducing organizational healthcare expenditures. This role supports a non-traditional care management approach by bringing "care for the caregiver" to the frontline to act as a health coach, navigator, and advocate, focusing on chronic disease management and preventive care at no cost to Covenant HealthCare employees.

Patient Assessment
- Conduct mobile health assessments by meeting employees in their respective workspace to minimize shift disruption.
- Perform biometric screenings (BP, glucose, BMI) and review recent lab work during brief, scheduled Wellness Huddles.
- Identify barriers to self-care inherent in the healthcare environment (ex. fatigue, irregular mealtimes, high stress triggers).
Chronic Disease Management & Education
- Develop individualized Health Action Plans for employees with chronic conditions (Diabetes, Hypertension, Heart Disease, etc.).
- Provide specialized education on medical device usage, such as Continuous Glucose Monitors (CGMs).
- Promote a motivating climate by framing health goals as a pathway to professional longevity and personal quality of life.
- Act as a liaison to help employees navigate internal health resources and coordinate specialist appointments, and community resources as needed.
- Provide employees with Health & Wellbeing resources Covenant offers (i.e. Hands of Hope, Employee Assistance Programs, Pastoral Care, Good Path, Home Thrive)
Administrative
- Collaborates with Benefits team as a resource to interlink employees with the Benefits at a Glance Booklet and refer employees to Benefits as needed.
- Acts as a resource during annual Employee Benefit and Health Fair(s).
- Educate employees on high-value, low-cost healthcare options, such as the on-site pharmacy, community resources, and options for onsite health coaching.
- Assist employees in fulfilling requirements for Premium Incentive Credits which help eligible individuals with low to moderate income afford health insurance purchased through the Health Insurance Marketplace, so they meet the necessary milestones to reduce their monthly insurance costs.
- Track and report outcomes of reductions in employer utilization and employee health outcomes.
Cross Departmental
- Participate in quality improvement initiatives and staff meetings, interlinking meetings and committees with Human Resources as needed.
- Maintains confidentiality of patient information according to established practice policies and government regulations, such as HIPAA.
- Accurately recording patient information, assessments, care plans, and interventions in the electronic medical record.
- Maintains professional competency through ongoing training and continuing education.
- Performs other duties as assigned. Qualifications:
EDUCATION/EXPERIENCE
- Associate's degree in Nursing from an accredited school.
- Three (3) years' experience in nursing required. Experience in care management, case management, chronic disease or occupational health preferred.
- Current licensure as a Registered Nurse in the State of Michigan.
- BLS certification required.
KNOWLEDGE/SKILLS/ABILITIES
- Proficient in computer applications, including Microsoft Office and EMR.
- Knowledgeable in chronic disease and occupational health management.
- Demonstrates appropriate care techniques.

- Ability to work independently or as part of a team.
- Effective and positive communication skills when interacting with patients and families.
- Exemplary customer service skills.
- Competent in essential clinical nursing skills.
- Maintains accountability for practicing within the scope of the Registered Nurse role.

WORKING CONDITIONS/PHYSICAL DEMANDS
- Constant travel to all Covenant HealthCare locations to provide on-site service to employees.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
- Frequent sitting, standing, walking, talking and hearing, pushing, pulling, stooping, kneeling, squatting, carrying, handling, tasting, and smelling.
- Frequent lifting 11-25 lbs.
